Where to find your batch number: it's printed on the bottom of the labels at the head of the mattress and starts with BD000_____ (e.g. PO# BD001327).

If there's no such number, attach a photo of the labels instead.

How to take your photos:

Bare mattress — no sheets or mattress cover
A ruler or string laid across the surface to show indentation depth
A ruler at center for dimension issues
A few full shots of the mattress, plus close-ups of problem areas
A photo of the surface the mattress rests on (foundation, platform, etc.)
